REQUEST TO OPEN PORTS ON THE FIREWALL

Please fill out this form if you need ports opened on the University Firewall. Only Internal Computer System Administrators or their Directors can request this function.  No ports will be opened without submitting this form. For security, all requests will be verified with a phone call confirmation.

Please note: Our firewall is currently programmed by our vendor. There is a charge for port adds, removes and changes to our firewall. This cost is currently absorbed by Network Services, therefore, we normally call the vendor to open a firewall ticket on a monthly basis after accruing the months requests. 

If your request is outside the normal time-line and it is urgent to open ports to your server, a ticket will be called in on your behalf. That ticket is counted as a normal request by the vendor and usually can be programmed within a week. Our vendor does not classify port programming as an emergency such as network down.  Please note that if your request is outside the normal submission time of Network Services monthly call-in (which will be the last work day of the month), your department will need to pay $135.00 for the programming services to the vendor when the invoice comes in. If the service is considered an emergency that is needed sooner than a week, the price will possibly be higher.

If you don't know the exact name of your service, please enter the description of what you are doing (such as File Transfer, Using PCAnywhere, etc (this is for documentation and security purposes.)) It is necessary for System Administrators to know the port numbers and protocols of the services they are opening to their servers.

Example: Telnet, Http, SSL, FTP, etc. If more than one service is needed, please separate by commas.

* What specific Port Numbers do you need and what protocol do you need for each specific port? Example: Port 80, UDP, TCP, or BOTH.
Separate each port request by a comma: Example: Port 80 TCP, Port 21 UDP, Port 28 BOTH
Put each port number request on a line by itself.
